Medical Release Form Printing Instructions

All head coaches must print a medical release form for each player on your team.  You will not be allowed to pick up your team's uniforms unless you can show BSC the stack of forms for your team.

The forms may be printed from your (head coach's) login on the registration system.

  1. Go to the registeration page and login.
  2. Look for your team management box toward the bottom of the page.
  3. Select the "view documents" link


  4. Select the "view medical releases" link



  5. Select the link for each player on the team and print the form
